What You Need To Register With Us
You do not need to provide ID, proof of address or proof of immigration status to register with us.
If you have your NHS number, this can help us locate your medical records more easily, but it is not required to complete your registration. To find your NHS Number click here.
As part of the process, we may also ask for:
• The name and address of an emergency contact
• The name and address of your current GP, if you have one
• The name and address of your carer, if you have one
In some cases, we may ask for additional information or documents. This could be to help us locate or transfer your medical records, or to confirm that you are the parent or guardian of a child you are registering.
If you do not have a permanent address, you can still register with our practice using a temporary address or our surgery address.
Frequently Asked Questions
There are several reasons why your registration may be rejected, such as living outside the catchment area, having previously been removed from our patient list, or providing insufficient information.
To help prevent this, please complete the registration form as accurately as possible and check all details before submitting.
If your registration is refused, we will write to you within 14 days to explain why.
We do not accept out of area registrations, please check you are eligble here before starting your application to join us.
It’s usually within 5 days of the surgery receiving your details, but it may take longer.
This option may be suitable if you’re living away from home but do not wish to permanently change your GP surgery, or if you’re visiting from abroad.
After 3 months, you can either re-apply as a temporary patient or register with us as a permanent patient here.
If your application for temporary registration is not accepted, we can still provide any necessary immediate treatment for up to 14 days.
If you do not have a permanent address, you can register with a GP surgery using a temporary address or the address of the GP surgery.
