How to book
To book a travel vaccination appointment you can:
• Use our Online Service (Patchs), Monday to Friday from 8:00am to 6:30pm. We will respond asap during opening times
• Phone us on 01484 440840, Monday to Friday from 8:00am to 6:30pm
• Visit the surgery and speak with a receptionist, Monday to Friday from 8:00am to 6:30pm
If you require travel vaccinations you must complete a travel vaccination questionnaire. This needs to be completed and given to reception. Once you have completed the questionnaire you will be given an appointment 2 weeks after. *Please note you will need to complete a travel questionnaire for all travellers.
It is important to make contact with the practice as early as possible - at least 8 weeks before you travel - Some vaccines have to be ordered as they are not a stock vaccine.
Some travel vaccines incur a fee because not all travel vaccinations are included in the services provided by the NHS. You will advised of fees by reception.
